Psychedelic 12-Step Program: How a New Approach Is Saving Lives
This paper reviews studies evaluating the use of LSD, peyote, ibogaine and ayahuasca in the treatment of dependencies and the possible mechanisms underlying the indications of effectiveness. Evidence suggests that these substances help assist recovery from drug dependency through a variety of therapeutic mechanisms, including a notable “after-glow” effect that in part reflects their action on the serotonin neurotransmitter system. Serotonin has been long recognized as central to the psychedelics’ well-known phenomenological, physical, emotional and cognitive dynamics.

In 2020, Oregan became the first-ever US state to legalize psilocybin, the psychedelic compound in so-called magic mushrooms, for therapeutic use. Denver, Colorado, and Oakland, California, decriminalized magic mushrooms in recent years. Altman also posted on X about psychedelics in 2022, evoking a response from Elon Musk, who is also vocal about support for psychedelics, about how “Amphetamines negatively affect empathy” and “psychedelics do the opposite.” The CEO didn’t specify which psychedelics he used, but he said the experience changed him in a lot of ways.

Yet the research (and funding) for psychedelic therapy as addiction treatment is growing. Addiction treatment centers and therapists may benefit from adding psychedelics to their toolbelt of treatments. And a modernized, more harm-reduction focused approach to AA is desired are psychedelics addictive by many in the community. Either because they find AA to be too restrictive, or like in the case of Ethan Covey, photographer and co-Founder of the Psychedelic Sangha group in NYC, they get the help they need from AA, but eventually outgrow it and are ready to move on.
Drug and Alcohol Dependence
- Kevin Franciotti, who’s involved in a similar group on the East Coast, Psychedelics in Recovery, that’s now mostly an online community, tells me many members of his group are seeking out psychedelics for similar reasons.
- Their symptoms were assessed at baseline and again at 1, 7, 14, and 21 days post treatment.
- Much like Bill W., they didn’t view it as taking a drug or as a breach of abstinence, but rather as medicine.
- And he’s right, everyone who struggles with addiction and substance misuse/abuse is on their own very individual journey.
- Some professionals have also turned to psychedelics and other substances like ketamine in an effort to enhance their work performance.
It really depends on the person and how they’re actively engaging these dopamine-releasing activities. And that’s another reason why support groups specifically for psychedelics in addiction recovery are so important, to help people navigate this tricky landscape and hold themselves and each other accountable. Dopamine has been extensively investigated as a biomarker for addiction and dopamine D2/3 receptors (DRD2/3) in the striatum have been quantified in several subclasses of addiction using the [11C]raclopride radiotracer. Findings from such studies have been mixed, with alcohol and stimulant use disorders appearing to show decreased DRD2/3 receptor availability when compared with matched controls.

This may take a few days to a week, and it’s often a physically and mentally painful process. Depending on the substance and use severity, inpatient treatment may be essential to keep patients safe during this phase. Indigenous cultures have long used plant medicines like ayahuasca, peyote, and iboga for healing and spiritual purposes.
Addiction and Mental Health Resources
We will then cover modern-day clinical trials of psychedelic therapies in addiction from first-in-human to phase II clinical trials. Finally, we will provide an overview of the different translational human neuropsychopharmacology techniques, including functional magnetic resonance imaging (fMRI) and positron emission tomography (PET), that can be applied to foster a mechanistic understanding of therapeutic mechanisms. A more granular understanding of the treatment effects of psychedelics will facilitate the optimisation of the psychedelic therapy drug development landscape, and ultimately improve patient outcomes. There is significant evidence for dysfunction in reward processing in addiction. One of the most common fMRI tasks employed in addiction populations which measures neural responses to monetary reward processing, is the monetary incentive delay task (MIDT) (85); which has identified pathophysiological reward processing in individuals with addictions. Meta-analyses from studies employing this paradigm have generally found hypo-functioning of the VS during the anticipation of reward in patients with several subclasses of addiction versus matched healthy controls (86).
Most psychedelics are still classified as Schedule I drugs in the United States, making research and clinical use complicated. Conventional addiction treatment approaches have long relied on a combination of behavioral therapies, support groups, and sometimes medication. These methods have helped countless individuals, but they’re not without their limitations.

These rely on the principle that psychostimulants, such as methylphenidate or amphetamines, can reliably increase levels of endogenous dopamine (161), opioids (162) and more recently serotonin (163). Non-pharmacological manipulations include stress, motor tasks, video games, and cue-induced craving (164). These findings highlight increased hypothalamus and substantia nigra DRD3 in alcohol dependence and stimulant use disorder, respectively, and have led to the development of novel interventions such as the DRD3 antagonist. In vivo molecular neuroimaging in the living human brain has been made possible by the advent of PET and Single Photon Emission Computed Tomography (SPECT).
- Experienced therapists are able to use their expertise in multiple therapy modalities to help patients integrate.
- You probably have lots of questions to clear up before you invest your time and money into education.
- Lysergic acid diethylamide (LSD) was first synthesized in 1938, though its psychological properties were not discovered until 1943 (15).
In my view addiction is best understood as a learning disorder, and best defined as repeating a behavior despite mounting negative consequences. It is easy to see how psychedelics could theoretically contribute to this kind of process, but in practice it is rarely seen. “This binary doesn’t take into account whether you’re using these substances to achieve or maintain a state of recovery, or to manage co-occurring disorders and health conditions, from depression to chronic pain and cancer,” wrote Olivia Pennelle. The prevailing sentiment in many 12-step groups is if a person doesn’t abstain from certain mood-altering substances (cannabis and psychedelics primarily), they are not in recovery. This isn’t for everyone – people with certain mental health conditions or a family history of psychosis may not be suitable candidates. It’s crucial to approach this with caution and under professional guidance.
This trend is seen globally with the WHO reporting increases in drug-related deaths in every continent of the world (4). Addiction is an economically crippling disorder exacting more than $442 billion annually in economic burden (5). This has far-reaching deleterious consequences that go beyond the individual, impacting employment, productivity, public health and the judicial-legal system.
